Physical Therapy Services in Kodiak
My Background: Physical Therapy is provided by Zoya Saltonstall. She has a Masters Degree in Physical Therapy from Ithaca College, NY.
Fees and Billing Insurance: Prior to your first scheduled appointment, call your insurance company to completely understand your physical therapy benefits. Below you will find an Insurance Verification Worksheet to help you ask your insurance company the right questions about your physical therapy benefits.
- Cost of your first visit for cash pay is $180.00. Payment is due before or on your first visit. We accept cash, check and credit card payments through PayPal.
- You will be billed for your visits within 14 days of treatment. I will be happy to provide chart notes or any other documentation at your-or your insurance company's request.
- All balances are due within 60 days from the date of service.
- If you have financial hardship and will be unable to pay in full, let us know so we can arrange a long term payment plan.
- We are a Tricare and Medicaid provider and are obliged to bill these insurances on the behalf of the client.
What should you expect on your first visit? There will be a bit of paperwork to fill out, so please
plan to arrive a
few minutes early. Zoya will ask you questions about your medical history, the condition you’re coming for and what
you hope to gain from physical therapy. Then she'll do an examination - taking a look at your movement, strength,
range of motion, muscles, joints, etc. The focus of the session will be on treatment, of course - helping
you to feel better! Another important component of the session is talking with you about things you can at home to
further your progress. Your input is a very much appreciated as we go about finding how you best recover,
strengthen, heal, etc. The initial session runs 1 hour and subsequent sessions are usually 30 minutes to 1 hour.
